PRN ~ Certified Nursing Assistant
Position Summary:
The Certified Nursing Assistant is an integral member of the interdisciplinary team and as such is responsible for providing, under the supervision of a Registered Nurse, personal care services to patients and/or homemaker services in support of the caregivers. The certified nursing assistant can be a Mercy Hospice employee or provided through a vendor contract. Mercy Hospice shall provide an initial orientation and training and shall supervise the certified nursing assistant as required by CMS regulations and the vendor contract. This position is an on-call as needed position. It can include evenings, weekends, and or holidays: but is not limited to those hours.

Others: Interacts with RN Primary Case Managers, patients and their families.
Patient Care Duties and Responsibilities:
Performs under a written care plan or verbal instructions from the RN primary case manager, personal care and personal hygiene for patients that might include: bathing, catheter care, active and passive range of motion exercises, assistance with feeding, use of lift as appropriate, bed making—occupied and unoccupied, assistance with ambulation or transfer as appropriate, and other CNA tasks as trained and assessed as competent.
Change dry, non-sterile dressings as needed.
Applies Duoderm and Tegaderm and engages in other individualized types of wound care after instruction and observation by the RN primary case manager or the RN wound specialist.
Records bowel activity but disimpacts and/or administers enemas only after individualized instruction and observation by the RN primary case manager.
Offers comfort measures for pain relief (i.e. cool, moist cloths to head; turning and repositioning the patient in the bed).
Maintains universal precautions at all times.
Accurately documents care and observations in a timely manner on the CNA care plan and service record.
Practices safety measure in giving patient care.
Maintains accurate records of activity when on duty.
Reports to Nursing Care Coordinator any abnormalities, any changes in patient condition and/or significant patient or family conversation or concerns.
Ensures clean, safe environment for patient by performing simple tasks such as sweeping floor in patient’s room, light damp mopping or dusting of furniture and rearranging of furniture for patient’s convenience and safety.
Maintains effective working relations with patient and members of the interdisciplinary team.
Maintains professional boundaries, actively listens to patients and families, communicates appropriate information to patients and families, maintains principles of HIPPA and confidentiality, and complies with safety, risk, infection control policies and procedures.
Non-patient Care Duties and Responsibilities:
Participates in at least 12 hours of in-service annually.
Maintains professional boundaries and maintains appropriate communication channels between the primary RN case manager, CNA supervisor, nursing care coordinator and other members of the interdisciplinary team.
Conforms to the agency dress code policy, in that no jeans are permitted.
Qualifications:
High school diploma or equivalent.
Completion of nurse’s aide certification course at an accredited school/program.
Must be certified as a Homemaker-Home Health Aide and in good standing with the Certified Nursing Assistant Registry.
One year experience as health aide preferred, but not required.
Knowledge, Skills and Abilities:
Must be empathetic and responsible, able to use time effectively, to accept supervision, and relate well to patients. Must be able to work on a team and be committed to the Hospice concept of care.
A knowledge of the core care standards and rules for hospice and palliative care.
A repertoire of service-driven and patient-oriented interpersonal skills, a sincere appreciation of people.
The possession of multifaceted skills accompanied by flexibility; the knowledge that change can be difficult.
Able to recognize stress-related responses and able to develop coping mechanisms to deal with the innate stress of providing services to persons in crises.
Ability to take direction from RN case manager and conform to directions given by the primary case manager.
The desire to continue learning and being open to new information and clinical skills.
The ability to be open and sincerely accepting of people’s unique and chosen lifestyles and the effects that these lifestyles have on their health.
The awareness and acceptance that a constant balance must be maintained between patient/family demands and the agency’s demands.
A sense of humor and practical wisdom.
Time-management skills to be able to prioritize and manage diverse and sometimes equally important tasks and responsibilities.
Working Conditions:
Works occasionally in a normal office environment where there is little discomfort due to noise, heat, dust, etc.
Some exposure to lifting and pulling of patients is expected. Must regularly lift and/or move up to 10 pounds, frequently lift and move up to 20 pounds, and occasionally lift and/or move up to 40 pounds.
Requires ability to interact with all staff members and participate in some community activities.
Requires extensive travel within Horry, Georgetown and Marion Counties in personal automobile, therefore, must have current drivers license, auto liability insurance and ability to operate vehicle.
Visits are made in patient’s homes which are not usually accessible to those who are unable to walk and/or climb stairs.
Contact with patient/families with a risk of exposure to communicable diseases; may be exposed to blood body tissues and fluids regularly, as well as other hazardous waste materials.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
While performing the duties of this job, the employee is regularly required to sit; use hands to finger, feel objects, tools and controls; reach with hands and arms; and talk and hear. The employee is required to stand, walk, stoop, kneel, crouch, and/or crawl.
